The following forms are available for download (PDF). Forms are fillable. Please complete, print, and return to the Financial Aid Office. (You will need Adobe Acrobat Reader to download and print the following forms. This free software is available for download from the Adobe website.)

Students who are selected for verification and were not eligible to use the IRS Data Retrieval Tool on the FAFSA must provide the Financial Aid Office with a copy of their Tax Return. If you elected not to use the IRS Data Retrieval Tool and were eligible to do so, we encourage you to log back into your FAFSA at https://studentaid.gov and link to the IRS.
